Players who did not participate inthe 2009 season mustpresent a birth certificate or other proof of age at registration.
1. Go to www.eAYSO.org and fill out a registration form. 2. Print and sign 3 copies of the registration form. 3. Bring the 3 copies of your registration form, birth certificate or other proof of age (if required, see below), and payment to one of the registration dates shown above.
Please note that your registration isNOT complete until forms and fees are received at theregistration site.
